Community Partnerships for Healthy Children Initiative
From
1993 to 2004, Center for Collaborative Planning provided technical
assistance, training and monitoring
for the Community Partnerships for Healthy Children (CPHC) initiative
funded by Sierra
Health Foundation.
CPHC promoted
community building as a way to improve children's health and
well-being. Our support helped build the capacity of 31
Northern California community collaboratives to problem solve,
mobilize resources and implement community-driven solutions. For

Building and Maintaining Collaboratives
CCP provided training and technical assistance to CPHC communities
to develop and maintain well-functioning collaboratives. A well-functioning
collaborative excels in 5 key areas: communication, decision-making,
facilitative leadership, inclusivity and participation, and maintaining
a shared vision. CCP designed a process for collaboratives to self-assess
their progress and create corrective action plans based on the results.

Asset-Based Community Planning
CCP coaches groups in "Asset-Based Community Planning," an approach that engages local residents to develop strategies by discovering and using local resources. CCP authored one in a series of guidebooks called "We Did it Ourselves" that contain stories, tips and activities to assist in the planning process. The guidebook outlines six steps in the participatory planning process: Visioning, Understanding Local Issues, Resource Identification and Mobilization, Strategy Development, Action Planning, and Creating an Evaluation Plan. Sustaining Collaboratives
CCP developed a tool to help CPHC collaboratives sustain their successful efforts. Our framework for sustainability consists of four components: policy and systems change, community mobilization, institutionalization or spin-offs, and fund development. This sustainability model builds upon work by Tom Wolff of AHEC/Community Partners.

CPHC Council
CCP
helped facilitate the development of the Community Partnerships
for Healthy Children (CPHC) Council. This council is composed of
18 community collaboratives who participated in the CPHC initiative.
During the development period, CCP and Public Health Institute (PHI)
acted as an "incubator" for this newly emerging regional
association.
The CPHC Council supports
grassroots, community-based collaboratives to work together to improve
children's health. Because the Council believes nothing is more
important than children's health, they are focused on policy issues
that improve health access for all children in California. Council
members attend statewide advocacy meetings, work to publicize best
practices for grassroots advocacy and hold an annual training at
the State Capitol to educate community members about how to impact
the political process.
