A Center of the Public Health Institute


Supporting and Managing Community Initiatives

Center for Collaborative PlanningCCP assists funders and community leaders in shaping innovative, sustainable community improvement efforts. Through coaching and modeling collaborative leadership, we help create learning communities, building members' capacities to plan, implement and self-assess progress. When contracted to manage initiatives, our unique approach combines technical support with monitoring functions. As we continue supporting grantees, we also monitor progress, ensuring communities are meeting their goals and those of the initiative.

Community Partnerships for Healthy Children
Since 1993, CCP has provided technical support to the CPHC initiative, a 10-year $20 million effort to improve children's health funded by Sierra Health Foundation. In 1999, CCP began monitoring grantee progress and managing the initiative's day-to-day business until its conclusion in 2002.

Our role included providing technical support to grantees to implement their strategies, assess their progress and comply with grant requirements. We also monitored the status of financial, self-assessment and evaluation reports.

CCP served on a leadership team with Sierra Health Foundation and SRI International (the organization conducting CPHC's formal evaluation) to work in partnership to develop, shape and sustain the initiative. With this team, we created a theory of change and conducted a Request for Application process.

Partnership for the Public's Health (PPH)
In 1999, The California Endowment (TCE) awarded approximately $37 million over five years to the Public Health Institute to plan and implement an initiative to stimulate the redesign of systems to protect and improve the public's health in communities throughout California.

CCP worked with the initiative's leadership during the planning phase, including conducting statewide focus groups to generate information to help shape the initiative. Since the launch of PPH, we continue in our supportive role by working with staff to develop relevant trainings, self-assessment protocols and a theory of change specific to this initiative.

Grossmont-East County Healthy Neighborhoods Initiative
Beginning in 1997, CCP worked in partnership with Grossmont Healthcare District, East County Community Clinic and community leaders to design the initiative and engage residents in the process of identifying critical issues, assets and strategies related to improving community health.

CCP facilitated a community-based study, organized a region-wide community visioning session and trained residents in asset-based community planning approaches. Sixteen neighborhood collaboratives were funded to continue planning and to implement locally developed solutions. The results of their efforts are captured in "Building Healthy Neighborhoods Collaboratively: Telling Our Story," an evaluation report sponsored by Alliance Healthcare Foundation.
